To be recognized as a Nonprofit Charitable Corporation under Section 501(C) of the US Internal Revenue Code, a corporation must file for an exemption.

Download - Sample IRS Form 1023 Application for Recognition of Exemption 501(c)(3)

Form 1023 that was filed by a Local Crime Stoppers program in 1996. The names and addresses have been changed to make this form into a sample. Excerpts of the Form 1023 Instructions were included with this sample. This sample is just one example of how a Crime Stoppers Program can complete the application. Your articles of incorporation and bylaws, and state laws will determine how you answer the questions on the form. Be sure to include all documents required in the instructions. See Form 1023 Checklist.
